It will about 8 to 10 minutes to do this survey. Your answers will help us understand how our community is doing and how we should plan for the future. Plus, we've got prizes you might win! 

Stuff to know...

We need your name and contact information for our prize draws and to check that you are a member of the Band. Your name will not be kept with your responses when they are sorted. If a staff member helps you fill out the survey, they have signed an agreement to keep your information confidential (private).

If you're filling this out online, you can skip some questions. Others you have to answer to keep going with the survey. In most of those cases you can answer "don't know / prefer not to say." Please answer as many of the questions as you can. The more we know, the better we can plan!
