CMSA relies on feedback from players, parents and coaches, to guide us in our planning for future seasons and events. The results will help to identify key themes, shape the delivery of our programs, and ultimately provide the most meaningful soccer experience for all. By filling out this survey, you will be eligible to enter a draw to win a prize of $100.

We would love to hear about the player's experience from the past season, regardless of age. Feedback is appreciated from players at 6 years of age, all the way up to 18 years of age. We ask you to kindly pass this survey along to them to complete in addition to your responses. Remember, each entry is a chance to win, so the more responses we receive from your household, the greater your chances are of being chosen.
