File Usage Approval Form

Hey guys, I have been asked by Seed Company to get written approval from each of you to use a portion of your video (Team Tuesday video) on my portfoliosite. I am not looking to make a profit off of it or sell it, I am using the video as a visual representation of the prior culture and teams I built to bring personality to my portfolio and not just use stock video.
