Thank you for registering as a guest. Before you are able to play with us we require you to confirm your agreement to the following waiver:
WAIVER RELEASE & INDEMNIFICATION
In consideration of the acceptance of my membership or participation in events with the Melbourne Smashers Badminton Club Inc.
I waive all current and future claims that I may have against the Melbourne Smashers Badminton Club Inc , its officers, directors, members, volunteers, employees, agents and sponsors, or its executors, administrators, heirs, successors or assigns (the organisers), and release them from all claims for death, injury or damage arising out of my participation in their club, their event and its related activities, together with any costs, including legal fees.
I agree to indemnify and keep indemnified the organisers against damage arising out of my participation in the events and its related activities.
I agree that I am solely responsible for any damages I cause to equipment or facilities and that I agree indemnify Melbourne Smashers Badminton Club Inc against any claims for said damage.
I agree to comply with all the rules, regulations and instructions of the organisers.
In the event of injury or illness during participation, I consent to receive medical treatment, which may be deemed advisable by the organisers or by the attending first aider and any subsequent medical professions. In the event of accident, injury or illness during participation, I consent to receive medical assistance and treatment including blood transfusions and anaesthetic, and I agree to pay all such fees and expenses incurred. I understand that this does not in any way affect my ability to make a claim against any insurance that I may have, including insurance as offered through Badminton Victoria.
I acknowledge that I have sole responsibility for my personal possessions and equipment during all events and activities.
I agree that Melbourne Smashers Badminton Club Inc. may use my name and picture in any broadcasts, telecasts and the press including website as they pertain to the Melbourne Smashers Badminton Club Inc and their events and activities. I may request Melbourne Smashers Badminton Club Inc to remove any image of me or use of my name by notifying a committee member in writing and that I will give Melbourne Smashers Badminton Club Inc a reasonable amount of time to make the amendment.
I understand that my membership or entry fee is non-transferable and not refundable.
I agree to have my personal details recorded and used by the Melbourne Smashers Badminton Club Inc for further communications of future related events. Melbourne Smashers Badminton Club Inc may pass, with my consent, my information onto third parties who require the information for the provision of goods or services that I acquire through membership of Melbourne Smashers Badminton Club Inc. Melbourne Smashers Badminton Club Inc will not pass my details on to any third party without my consent. I agree that Melbourne Smashers Badminton Club Inc will pass on my details to Badminton Victoria for the provision of insurance and may pass my details onto any attending first aiders or medical professionals where that information is required to assist in the provision of first aid or medical assistance.
