Use this form to request your login for the Wellness Workbook for Early Care and Education by Nemours Children's Health. If you are using the Wellness Workbook as part of a local or state initiative and will receive technical assistance from a Trainer/Coach, your program assessment information may be viewed by your assigned trainer. You will be notified in advance of your assigned Trainer, if applicable. Nemours Children's will not share or sell any of your individual contact information with a third party or for any commercial interests. You may receive e-mails from Nemours Children's regarding the Wellness Workbook and related updates or resources released.  

Once you complete this form, you will receive your login within 24-48 hours. If you don't receive a login within 72 hours, then email workbook@healthykidshealthyfuture.org
