A Community Language School (CLS) may nominate one person per category (a maximum of 3 nominations in total). You can use this form to make one nomination in one category. To make nominations in multiple categories, please submit the form again.
For example, nominate someone for the Language Program Teacher award, then submit the form again to nominate someone for the School Teacher award.

Note that each nomination form must be endorsed by two members of the CLS leadership team, e.g. Principal, School Administrator, school management committee member or school governing committee member. This endorsement can be given by signing the Nomination Endorsement form. Collect signatures using this form and upload it in the final step of the nomination submission.

Nominations close Monday 4th November. Late submissions will not be considered.
