West Virginia Immunization Network Annual Awards: Recognizing Excellence

Seeking Nominations for WIN’s Annual Awards Event

The West Virginia Immunization Network’s (WIN) mission could not be accomplished without partners from the public and private sectors whose tireless work and innovative strategies have improved immunization coverage levels throughout West Virginia. To honor these essential partners, WIN seeks nominations for our annual awards to recognize excellence in immunization efforts over the past year.

Eligible nominees include any individuals, clinic, organization, community group, professional association, or corporation whose work has helped further the mission of WIN and improve the health of West Virginians. Nominees and nominators need not be members of the WV Immunization Network.

Awards may be given in, but are not limited to, the following areas:
- New Program or Initiative;
- Use of Immunization Registry;
- Community Outreach Campaigns in Childhood, Adolescent, or Adult Immunization;
- Advocacy Effort or Policy Development; and
- Special Achievement.
The most outstanding nomination will receive the 2023 WIN Hot Shot Award.

Submission of Nominations:

To nominate an individual or organization, please complete the form below. The Nomination Narrative should detail how the nominee has demonstrated excellence in immunization efforts over the past year. Please be as descriptive as possible, as you complete section #12. Nominations may be accompanied by supporting materials, which may be emailed to lindsay.barr@wvruralhealth.org.

Selection Process:

Nominations will be reviewed by a panel of Steering Committee members. Nominators will be notified if their nominee has been selected prior to the awards ceremony. Award recipients will be presented with their awards at the WV Immunization Summit, during WIN’s Annual Awards Luncheon, scheduled for June 09, 2023 from 11:45am to 1:15 pm at the Summersville Arena and Conference Center in Summersville, WV.

Due Date:

Award nomination materials must be received by The Center for Rural Health Development, Inc. by April 17, 2023 at 11:59pm.
