Introduction
The Warren Health Equity Zone (HEZ) collaborative is working to better understand the needs of Warren residents and others who learn, work, and play here. Please help us by filling out this Community Survey.
This survey has questions that are multiple choice, and others that allow you to write more detail. There is also a section about you and your household. It will take about 15 minutes to complete.
Warren HEZ supports resident-led projects related to health and wellness. Health Equity Zones are an initiative of the Rhode Island Department of Health. East Bay Community Action Program is the backbone agency for the Bristol, Warren and East Providence Health Equity Zones.
