Dear Participant:
This letter is a request for you to take part in a research project designed to learn about perceptions of and responses to novel the Coronavirus (COVID-19). This research study is being conducted by Principal Investigator Hailey Gillen Hoke, Ph.D. at Weber State University.
This research is being conducted as part of a study examining COVID-19. In order to participate in this research, you must 1) be 18 years of age or older, 2) be a US citizen currently living in the United States, 3) be currently working remotely, and 4) not have been working remotely before the pandemic began.

Please complete the following questionnaire. Read each question carefully. If you are unable to answer a question, or do not feel comfortable with the question, please leave the response empty. There is neither a right nor wrong answer to any question. You may answer each question with as much or as little detail as you like. You may stop completing the questionnaire at any time.
Your involvement in this study is anonymous and your responses are confidential. Participation in this study is voluntary. You can stop at any point without fear of penalty. While there are no risks associated with participation in this study, some questions may bring up uncomfortable feelings or memories. Completing the questionnaire indicates that you have agreed to participate in this study. This questionnaire takes about 10-15 minutes to complete.
