Currently, there is an unmet need for quality bereavement education in nursing homes, not just for residents and families but for staff as well. We are also aware that the COVID pandemic has changed how you as nursing home caregivers provide certain aspects of end of life care. We are seeking feedback from you regarding current best practices in your homes.

We ask that when responding to this survey you answer to the best of your recollection PRIOR TO THE COVID EMERGENCY. Your time and response is very much appreciated.
1.Does your home have the following in place for providing person centered, person directed end of life care?
2.How does your home support it's staff during and after the death of resident, such as a dedicated space for reflection?
3.How does your home support the families of deceased or dying residents? (In addition to help with Advanced Care Directives and Funeral Arrangements)
4.How does your home support your other residents after the death of a fellow resident?
5.What type of training do you receive or offer staff around resident directed, end of life care and facility norms when a resident is dying or has died?
6.Does your home have dedicated private space for the resident & families during the dying process?
7.How do you keep families informed and prepare them for the death of their loved one?
8.Does your home have a process for gathering or sharing feedback from residents and families on your end-of-life practices?
