BLOCK #1: INTRODUCTION OF SURVEY

TteS Education Dept is conducting an important survey with community members 15 years of age and older who live in the community.  We were hoping to learn about community members’ employment status, work experience and training needs.   It’s our hope that we’ll receive feedback from our people that will help us know which training and education programs we should bring into our community based on community need.  
We will ask questions about your previous work experience, and some of your training or education needs . It takes about 15 minutes to do the questionnaire.  We have it set up on line using survey monkey or you can complete it with me in person.

The information that you tell and share with me will remain confidential. This means that only the survey team and relevant members of TteS Education will have access to the data that identifies you with your responses. 

Any reports that result from the survey will have the responses grouped together, so no one reading the report would be able to identify you or your responses. You name will not be used. Overall reports using grouped data could be used to help assist with applying for external funding, so we do appreciate your responses.

After all the community surveys are complete and the training and employment data correlated, there will be a community gathering to provide a summary report for community. Once again there will be no names shared and all information will be held confidential and anonymous.
