Rural Community Outreach Survey

As part of the Community Media Outreach program, the Department of Health and DH want to support partners in your outreach to rural communities. The aim of this survey is to better understand partner needs and challenges.  
  
This survey will take 5 minutes to complete. It is a mix of multiple choice and open ended questions. The findings will help DH identify opportunities to reach rural audiences and improve toolkit materials to better support partners’ rural outreach efforts. Answers to these questions will not impact future funding decisions.
