Would you like to be involved in changes and innovation in mental health care in your area?
If you're interested in using your lived experience to help your local PHN or other health service providers improve mental health services please take a moment to complete this survey.
We also explain a little about the role of primary health networks (PHNs) in relation to health services in the community.
