Recognizes a member whose program or project advances the evidence-based strategies inspired by ADCES's Vision, Mission, and Values.

AWARD
Recipient will be recognized at the ADCES Annual Conference with a plaque, will receive a complimentary registration to the Annual Conference and a $1,000 travel stipend.
ELIGIBILITY
Applicant must be an active member of ADCES, as defined by the Association's Bylaws, for at least three (3) consecutive years at the time of submission and receipt of the award. Past recipients are not eligible to apply for this award.
CRITERIA
Applications must meet the following requirements to be considered:
  • The educational project or program for use in practice must reinforce the ADCES Vision, Mission, and Values. 
  • The education project or program must be developed for use in practice DSMES.
APPLICATION PROCESS
Upload a description of your program or project.

This should include:
  • Goals and objectives, methods, program's needs and expected outcomes
  • Description in 1,000 words or less how your project or program aims will achieve the outcomes inspired by ADCES's Vision, Mission, and Values.

Applications and supporting materials must be submitted by March 3, 2025. All applications must be complete to be considered. Please provide as much detail as possible in your answers. Applications demonstrating thoughtful, detailed responses tend to yield more favorable results. If a confirmation has not been received by March 22, 2025, it is the applicant's responsibility to contact ADCES at awards@adces.org or call (800) 338-3633 x4810.
