Application Instructions

PURPOSE
Recognizes a diabetes care and education specialist for their on-going dedication and service to persons with diabetes and the impact of diabetes self-management and clinical care that diabetes care and education specialists provide.  The recipient of this award would be honored for their day-to-day commitment to clinical care to ensure the best outcomes for persons with diabetes. This award provides an opportunity for ADCES to recognize members in the field for their dedication to persons with diabetes who work every day to improve the quality of their patients' lives.
AWARD
Recipient will be recognized at the ADCES Annual Conference, receive complimentary registration to the Annual Conference, a one-year active membership, and $1,000 travel stipend.
ELIGIBILITY
Applicant for this award will need to have completed a minimum of ten (10) years' experience as a diabetes care and education specialist at the time of submission of the application. Applicant will need to provide examples of collaboration between the applicant (working in an inpatient or community setting for example) and the person with diabetes using a unique strategy or intervention which empowered the person with diabetes to make concrete behavior change, resulting in improved outcomes.

Applicant must be, a member for at least three (3) years of consecutive membership at the time of application, an active member of ADCES as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award. Current and past members of the Board of Directors and past recipients are not eligible.
APPLICATION PROCESS

CV or resume

Two (2) letters of recommendation, of not more than 500 words, from a professional colleague, to include:
  • How the author knows the applicant
  • Why the author is supporting the applicant for the Power of Our Impact Award
  • Provide examples of collaboration between the applicant (working in an inpatient or community setting for example) and the person with diabetes using a unique strategy or intervention which empowered the person with diabetes to make concrete behavior change, resulting in improved outcomes.
Current members of the Board of Directors and the Awards Committee are not eligible to provide letters of support for award applicants.


Applications and supporting materials must be submitted by March 3, 2025. All applications must be complete to be considered. Please provide as much detail as possible in your answers. Applications demonstrating thoughtful, detailed responses tend to yield more favorable results.
