Informed Consent

If you would like to discuss matters related to your health and wellbeing (or that of your dependent), this
confidential referral form can link you with the appropriate staff, program or assistance locating outside
resources.

We are committed to confidentiality.

The Referral Form will be reviewed by trained staff and
stored in a locked filing cabinet.

All information will be shredded 18 months following completion of services.
