School/Entity Field Trip Request Form

This form is intended for educational groups including schools, extracurricular groups, homeschool co-operatives, etc. If you are a non-educational organization, or other group seeking an educational experience, please contact our Ticketing & Reservations Department at tours@imsmuseum.org to discuss reservation options.

Please note - to qualify for field trip pricing, your educational group MUST include 10 or more students. Adult chaperones are required at an 8:1 ratio and must be over the age of 18.

To request a field trip, please complete the form below. Incomplete or unsubmitted forms will not be processed, and no confirmation will be sent. If you have any questions, please contact our Ticketing & Reservations Department at tours@imsmuseum.org.

Primary Contact Information
Each field trip or reservation at the Indianapolis Motor Speedway Museum must have a designated primary contact person. This individual will be responsible for:

Receiving and distributing all essential trip information.
Receiving and managing payment details for processing.
Assisting with logistical planning and coordination for the group.

If multiple contacts are involved, one primary contact must be designated to ensure clear communication.
