The OHSAA sponsors a tournament in each of the seven football divisions. The first two rounds are hosted by the higher seeded team and do not require this survey to be completed. Please complete this online survey only if you would like to have your site considered for one or more Regional Semifinal (round 3) through State Semifinal (round 5) games. By submitting this survey, you are agreeing to notify the OHSAA office immediately if the field becomes unplayable.
If this form is submitted multiple times, the most recent submission will be used for site selections. Once submitted, you can email any changes to Charles Anderson, canderson@ohsaa.org, instead of resubmitting the survey.
