Professional Practice Site Profile

Thank you for taking the time to complete this Professional Practice Clincial Site Profile. The purpose of this Clinical Site Profile questionnaire is to provide information regarding the physical environment, type of clinical experience, staffing and numbers of students that may be accommodated by your facility. The information is required for site approval by the university and for accreditation purposes. Please complete a separate profile for each site. Information that will be made available to the students is indicated in each section. Answers are required for each question with an asterisk. Please note that you will require information before starting this survey including: immunization and other requirements by your facility, facility dress code and budgeted Physical Therapist FTE information.
