Introduction
Thank you for taking this Continuing Medical Education (CME) Needs Assessment Survey from the SGIM Adults with Chronic Conditions Originating in Childhood Task Force. All questions are in context of Improving Transitions of Care for Adolescents/Young Adults with Chronic Conditions Originating in Childhood.
The survey should take only 5 minutes and will help us develop educational opportunities for the SGIM membership at large. Your answers will be completely anonymous. If we report the survey results, all answers will be presented as grouped averages.
In appreciation of your time, we will randomly select two respondents to receive $25 Starbucks gift cards. If you would like to be entered in the drawing, we need you to provide your Name and E-mail address at the end of the survey. This is only for the drawing; your name will not be associated with your answers in reporting survey results.
The survey is in two parts:
- Section 1 asks about your educational needs in regard to young adults with chronic conditions originating in childhood;
- Section 2 asks some demographic questions to help us understand the educational needs of the internist.
