What does participation involve?
You are invited to participate in a webform that is expected to take 5 mins. Your participation is voluntary. You are free to withdraw at any time up until the 15 of November 2024, when your information will be de-identified and merged with others. If you do decide to discontinue, all your feedback will be excluded from any further use.
Your consent
If you wish to participate, Terria ethics protocol requires your formal consent by checking the ‘I give my consent’ box in this web form.
Are there any risks?
Aside from giving up your time, there are no foreseeable risks associated with participating in this survey. If you give consent to participate, we will register you in a secure internal database with your supplied details and the results of the webform.
Confidentiality
All information provided by you will be treated confidentially. Data you provide us will be recorded through the use of Qualtrix. Any information collected in this study will be securely stored per Terria’s Record keeping Procedure.
How will my information be used?
Terria may prepare reports and presentations using the de-identified information you provide. This report will only be shared with the Terria and it’s shareholders. Your personal information will not be used or disclosed by Terria without your consent, except in accordance with Terrias privacy policy www.terria.io/demo-privacy-policy
All results are used to improve the technology solutions we develop.
