This is a volunteer survey that was created to get feedback from people with intellectual and developmental disabilities (IDD), their family members, and/or caregivers. The purpose of this survey is to find out what you think is most important to empower individuals with IDD with the necessary skills, knowledge, and resources to navigate adult life successfully. Please complete this survey to the best of your ability from your personal experience. If there is any question you are uncomfortable answering, you may skip it. By submitting my answers to the survey, I agree to participate.

If you would like to have a paper copy of the survey mailed to you, send an email to sharon@teamdyn.com or call 985 624 3514 and ask for a paper copy of the Transition to Adulthood Survey.
