SURVEY CONSENT
You are being asked to take part in a research study designed to gauge your perceptions related to five different dimensions of wellness. We also will ask questions about different wellness resources and factors that might inhibit or facilitate your ability to implement wellness practices in your own life. You are being asked to take part in this study because you are a current faculty or staff member at Truman State University.
You can choose to complete a 15-20 minute survey that involves a series of questions. It is possible you might experience some embarrassment or emotional discomfort when answering the survey questions. We do not know if you will benefit from being in this study, but we hope that, in the future, other faculty and staff may benefit as we would like to use the results from this study to provide better wellness programming for faculty and staff at Truman State University.
After completing the survey, you will have the option to enter a drawing to win one of two $50 University bookstore gift cards. If you would like to enter this drawing, please be prepared to provide your Truman email address. We will contact you the week of November 1st to notify you if you have been selected for the drawing. To help protect your confidentiality, we will destroy your personal contact information after we have finished this part of the study in May 2025.
The information you provide during this research study will be kept private to the extent granted by law. As we collect survey data, we will store the results on a computer that requires a password to login. The computer is kept in a locked university office. The data will be kept for forever. You may skip any questions you want, and you may choose to stop the survey at any time. You must be 18 years or older to participate in this study.
