The Reading Agency is a UK-wide charity working to empower people of all ages to transform their lives through reading. We are carrying out research into people's experiences of accessing, reading and using health information.

We are using the phrase 'health information' in its broadest sense. For example, that could be information related to a specific health condition, to types of symptoms, or even to more general information about lifestyle or keeping well. You might access that information in a book, eBook, audiobook, leaflet, article or online.

The questions should take around 7-10 minutes to complete. By filling in this survey, you will also have the opportunity to enter a prize draw for a £25 Love2Shop voucher, which can be used at a range of high-street locations. You can fill in this form until 29 April.

We may use extracts from your comments in our communications with partners but all individual responses and comments will be kept anonymous. Your answers will help us understand more about people's experiences of using health information to try and make our work better in future.
