2024 Disability Equality Index Award Ordering Form

Congratulations on earning a top score on the 2024 Disability Equality Index. Through this form you can order custom Disability Equality Index awards to display in your offices or to distribute to key leaders and contributors. This form allows you to order award(s) for one country at a time; if you wish to order awards for multiple countries, please complete a form for each country for which you are ordering. Custom awards are made available to be ordered at your expense through RED Swag, a Disability-Owned Business Enterprise (DOBE), that is certified by Disability:IN. The cost per award is US $45.53 + shipping ($15 in the United States, $25 internationally).

All orders must be received by Aug. 31, 2024; RED Swag will contact you in September to finalize payment processing and will ship awards by the end of September.
