LUMA Health Consulting has been contracted by the Maryland Department of Health’s State Office for Rural Health to conduct an impact assessment of the Total Cost of Care (TCOC) model on rural healthcare environments in Maryland. As part of LUMA's qualitative data collection, we are distributing surveys to relevant state partners, in addition to in-depth key informant interviews. The final deliverable will be a highly consumable report of findings that include the actionable and potentially replicable aspects of the TCOC model in rural areas. You may access this public report in July 2023 through the Maryland Department of Health's State Office of Rural Health's website here.
The five domains TCOC aims to improve are: (1) access and continuity, (2) care management, (3) comprehensiveness and coordination, (4) patient and caregiver engagement, and (5) planned care. Questions will focus on these domains, with additional questions on the financial impacts of the Maryland Health Model and observable trends.
