Survey Information for consent
This survey is going to ask for your experience and opinion about RSE you received and what you think is relevant and important for people around your age to know. We want to make sure that your voice is included in the current conversation around this topic. Please read the information below to decide if you would like to consent to participate.

To participate in the survey the following statements need to be true:
- You are 16-20 years old
- You have attended, or are currently attending, secondary school in New Zealand
- You recall receiving some relationships and sexuality education (RSE)

Please note that you are welcome provide your email at the end of the survey to enter a draw for one of ten $50 Prezzy cards!

Purpose
We want to know what you think and how you feel about relationship and sexuality education (RSE) in Aotearoa New Zealand. The information gathered will be summarised in a report and shared with people who are interested in this topic.
Procedures
This online survey will ask questions about your experience and perspective on relationship and sexuality education provided in school and from other sources. It is expected that the survey will take around 15-20 minutes. You do not have to answer any questions you don't want to.

Risks/discomforts
Content warning: the survey includes questions about whether you have been taught about topics including consent, sexual harassment, sexual abuse, pornography and relationships. Some of the questions may be difficult or sensitive to answer. You do not need to answer anything that feels uncomfortable and you can skip any question you do not want to answer or stop the survey entirely at any time. You answers will be kept confidential.

Benefits
There are no direct benefit to you, but your answers will help us understand more about what young people think about RSE in New Zealand.

Extent of Anonymity and Confidentiality
All your responses will be kept confidential. Your name will not appear anywhere in our research. No one will be able to trace your answers back to you. Each person who takes the survey will be assigned a number (e.g. 0210) which will be used to organise the data for the analysis. Select staff members from Sexual Wellbeing Aotearoa will have access to the data which will be used for a report summarising the findings and other possible academic publications. The data will be securely stored for a period of five years and then destroyed.

Compensation
You will not be compensated for your participation in this survey. You can decide if you would like to provide your email once you have completed the survey to be entered into a prize draw for a $50 Prezzy card.

Voluntary Participation
You do not have to agree to be part of this study. You may stop participating at any point during the survey or after the survey is complete, you may state that you do not want your responses to be included in the data. You also do not have to answer any questions in the survey for which you do not want to provide a response. You may contact Sexual Wellbeing Aotearoa at poliicy@sexualwellbeing.org.nz if you have any questions or complaints about being in this study or wish to have your responses removed from the data.

This survey has been adapted from the Sex Education Forum's Annual Youth RSE Poll conducted in the UK.
