2025 LBBC Survivorship Series Program Leader Application

Thank you for your interest in the Survivorship Series.

This application will take between 20-30 minutes to complete. The application deadline is Friday, December 6.

LBBC accepts applications from:

Healthcare providers who, if selected, would be a solo/individual Program Leader at their cancer center. Solo Program Leaders must be an RN-level oncology nurse navigator or oncology nurse.
We also accept team applications from two healthcare providers who work for the same health system/cancer center. If selected, these team members would work together to implement the Survivorship Series. At least one team member must be an RN-level oncology nurse navigator or oncology nurse. The other team member can also be an oncology nurse navigator or an oncology nurse, or could be another type of healthcare provider such as a patient navigator, community health worker, or social worker. If you are applying as a Program Leader team, please complete this application together. Only one application should be submitted.
