HORAN Campus Health, in collaboration with BHAC, appreciate your participation in this short Student Return to Campus survey.  

This survey is intended to:

1.  Provide BHAC members and friends of the consortium an opportunity to receive information on what other colleges and universities are including in their COVID-19 Student Return to Campus Plan

2.  Understand the implication of COVID-19 on student health benefits now and in the future
