Thank you for expressing your interest to volunteer to build a playground for Snaw-Naw-As community with Hope and Health as the project leader.

This project will be taking place Wednesday August 31st through to Friday September 2nd, 8:30am to 3:00pm daily.

Please indicate your availability, any dietary considerations and also complete the liability and consent sign off.

I acknowledge that building a playground may involve physically-demanding activity which can result in injury. I, for myself and the player and our respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify Hope and Health for Life Society, its officers, directors, employees and volunteers from and against all claims, liabilities, damages or causes of action arising out of or in connection with my participation in this volunteer activity.

I have read the waiver, and agree to abide by the, as a condition for volunteering in this project.

I hereby consent to and authorize the use and reproduction by Hope and Health for Life Society of any and all photographs and/or video that have been taken of the volunteers and project in progress, for any purpose, without compensation to me. Hope and Health reserves the right to use these photographs and/or video for social media promotion.
