SUAV Incident Reporting Form Introduction
Thank you for taking your time to complete this form. Your experience is important. This reporting form is part of JASMYN's Speak Up Against Violence (SUAV) Project. Violent incidents directed at the Lesbian, Gay, Bisexual, Transgender, Queer, Questioning, Intersex and Allied (LGBTQIA) community of Northeast Florida may often go unreported to authorities, and when they are reported, those incidents that involve members of the LGBTQIA community are often not clearly identified as such in local data.
Our SUAV Project's hope is to hear from our LGBTQIA community more directly when incidents of violence occur so that we may better understand the extent to which violence is impacting our local community. We want to know when there are violent incidents such as homicide and assault that impact our LGBTQIA community. We want to be informed about hate violence, intimate partner violence, sexual violence, and hook-up violence. We want to know about incidents of intimidation, bullying, and harassment. We want to hear about incidents of discrimination.
We also want to hear from non-LGBTQI persons if they have any incidents that have an LGBTQI connection such as a straight ally has their rainbow flag torn down, or someone directs anti-LGBTQI language while making a threat towards you even though you are not LGBTQI.
We hope your responses will help us identify any trends, inform us how our community should develop programs and services in response, and give us data for further advocacy with victim's service providers, law enforcement, and violence prevention specialists.
Your responses here will be anonymous, unless you wish to provide your information so that someone from the project can reach out to you to discuss your incident and provide resources. If you do provide your information and the incident you reported involved persons under the age of 18 or elder abuse, we will be required to report it to the Florida Abuse Hotline.
Reporting an incident to SUAV is not a substitute for reporting to 911, law enforcement, a school official, an abuse hotline, the Jacksonville Human Rights Commission, or any other sanctioned governmental agency.
We understand some violence occurs in the context of a pattern of abuse. For the purpose of this reporting form, please use the most recent incident when reporting bullying, harassment, domestic violence or other types of incidents that are ongoing.
We will be providing a list of resources at the end of the reporting form that may assist you if you have experience or witnessed a violent incident or case of discrimination in the Northeast Florida area.
