Understanding the Impact of Glucose Monitoring on Diabetes Outcomes in Adults with Type 2 Diabetes |

Please note that if you completed this brief questionnaire many months ago and failed to qualify for the study, the criteria have changed. So, it may be worth your while to complete it again and see if you might now qualify.
The purpose of this research study is to learn more about how different types of glucose monitoring devices affect people’s experience with the stresses and strains caused by diabetes. If you decide that you’d like to participate, you will be asked to complete the following steps:
- Complete the initial screening questionnaire below.
- Complete an “at-home” HbA1c test that you will need to mail back to us to determine if you are eligible to participate in the study.
If you are determined to be eligible and choose to enroll:
- You will be randomly selected to use either:
- a standard capillary blood glucose device for a three-month period, or
- a continuous glucose monitoring device for a three-month period
- You will be asked to complete an online survey twice – once at the beginning at the study and again 3 months later.
- You will be asked to complete a second at-home HbA1c test when you complete your second online survey.
The devices and all supplies for the three months will be provided to you at no cost.
You will receive a $50 electronic Amazon gift card at the end of the study, after completing both surveys and the HbA1c tests.
