Feedback from a participant’s support network

Woden Community Service (WCS) recognises, respects and values the significant role that family members, friends, clinicians and other people in a participant’s support network play in their recovery journey.

As a valued member of that support network, thank you for agreeing to provide feedback about the Mental Health and NDIS Services at WCS.  This feedback will help us to know what has been useful during the participant’s time in the program and how we can improve our services into the future.

We will keep the information you provide us confidential and use it only for the purpose of evaluating the program and improving the service. This brief survey should take less than 5 minutes to complete.
