Dining Out Program
We would appreciate you taking a few minutes to answer the following survey.
This survey is completely anonymous (unless you wish to include your contact details) and will not affect your membership or support from Coeliac New Zealand.
From this survey we hope to develop an understanding on how we are better able to support our members when dining out and how important this is within your daily lives. The following questions are designed to help CNZ to better understand the impact that dining out away from the home has on your health and wellbeing in terms of time management, social engagement and you work and family life.
Privacy Statement:
This survey is being conducted by Coeliac New Zealand Incorporated staff and volunteer students assisting with the project to gain a better understanding of people with coeliac disease eating out behaviour in New Zealand.
Participation is voluntary and for those over the age of 16. We will publish the results of the survey without any unique identifiers and it will be shared via our website and other media channels once complete. We are collecting generic information from you, including information about your age, gender and ethnicity for statistical analysis, however, completion of this survey does not require unique identifiers. We have asked for further information including your name, email and phone number as an optional inclusion. This information will only be used should we need to contact you regarding this survey. We keep your information safe by storing it on password protected databases.
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be correct if you think it is wrong.
