PLEASE READ CAREFULLY BEFORE COMPLETING THE SURVEY


If your child is attending an OUSD TK, K-12th grade this fall 2024 please reach out to your school site.


Upon receiving these items, Diagnostic Center must reply within fifteen (15) calendar days with a written
Assessment Plan (AP) for your child, or Prior Written Notice (PWN) letter explaining why an assessment will
not be conducted.
Please note that this timeline does not apply during summer recess, or school holidays exceeding five (5) days (e.g. spring break, summer break, winter break)


The following survey provides us important information that helps us assess your student for their eligibility for special education. The survey will take 30-45 minutes to fill out and will require you to upload pieces of information that are required to receive an assessment. All information we are requesting is important in determining the assessment team we assign to your child.
