2025 CSB Spirit of Excellence Awards Nomination Form (Team)

Nominations for the 2025 CSB Spirit of Excellence Awards will be accepted through Friday, March 14, 2025.

This nomination form may be used to nominate a team. Please complete the form clearly and concisely. This is the only information by which the nomination will be rated. Your nomination will be received once you click the "Submit" button at the bottom of the page and reach the confirmation page.

The nomination will be rated on a scale from 1-5.

1 - Insufficient information to support a rating.

2 - The nomination provides limited supporting evidence.

3 - The supporting information provides clear evidence but lacks nuance or laudable distinction.

4 - The nomination provides clear evidence of the nominee demonstrating support of the category characteristics.

5 - The nomination describes behavior that substantially exemplifies the category characteristics.
