The purpose of this survey is to explore the awareness of, knowledge about, and available resources for persons affected by domestic violence, sexual assault, and stalking in your community. The information gathered in this survey will be used to identify awareness about this public health problem and any unmet needs around this issue. Your input is vital to our understanding of how to better serve victims and their families, and to assist you in addressing needs in your workplace. This survey is will take approximately 30 minutes to complete. You can withdraw at any time during the survey process if you desire to do so. Neither your name nor contact information will be tied to your responses, therefore you cannot be identified as a participant in this needs assessment. The findings from this survey will be shared in a number of venues to assist you with problems you might identify when taking this survey.  You will be agreeing to participate by completing the survey.
