Please read and complete all items in the below form to ensure your Surf Sports Coaching Re-Accreditation is successfully submitted for processing.
Please allow up to 3 weeks for your re-accreditation to be processed by SLSQ. Status of your accreditation can be checked via awards section of your SLSA Members Area account.

Please refer to the Coaching Section of the SLSQ App, in particular the table on page 1 of the reaccreditation form (below), for reaccreditation requirements.
