Archbold is currently in the process of gathering input for our federally required Community Health Needs Assessment. Your feedback and participation in this survey will help us identify the community's greatest healthcare needs. We will include your input in the needs assessment which will determine our approach to providing assistance as appropriate. The information you provide will be anonymous which means no one will be able to connect you with the answers you provide. You must be 18 or older to participate.
