Apprentice Interest Form

Thank you for your interest in the DSP Apprenticeship Program, sponsored by The QuILTSS Institute! This DSP Apprenticeship opportunity is a work-based learning model where, as an apprentice, you will receive supervised on-the-job training, coupled with job-related online training, to ensure you have the skills needed to be successful in your role as a Direct Service Professional (DSP). The apprenticeship is available to both new DSPs and those already working with a participating employer partner. As you complete training and work hours, you will earn increased wages. Once you have successfully completed the one-year program, you will also earn a nationally-recognized credential as a QuILTSS-certified DSP. Please fill out the brief interest form below and we will get in touch with you shortly.
