SDP Student PT Agreement

* By providing my electronic signature below, I hereby agree to and understand the following:

I am solely responsible for getting myself to class on time following a physical therapy appointment.

I will not take photos of any kind while in the Boston Ballet physical therapy space.

I will not bring food or beverages (other than water) to the physical therapy space.

I will respect the privacy of Boston Ballet Company members and my fellow students.

I am responsible for returning any borrowed equipment prior to July 26, 2019. If I still owe any equipment on July 26, my parents or guardians will receive communications from SDP Staff asking for its return or for money to replace the item.

I understand that failure to follow any of the above rules will result in my physical therapy privileges being revoked.
