2. A lucky winner will be selected every 4 months at random
3. Only 1 survey per Order placed at Domino's Pizza will be accepted
4. Only fully completed surveys are accepted
5. Prizes are valid for a period of 12 months from issue date
6. Prizes are valid for in-store use only (not on delivery)
