Did you ride out the storm?
Welcome to the SUNY-ESF Hurricane Sandy survey for those who rode out the storm.
You will be asked to provide preliminary information (numbered #1-2) and answer four open-ended survey questions (numbered #3-6) about the time period leading up to the evacuation warning, and to review your thoughts and experiences at that time back in 2012. This will take approximately 15 minutes (more if you have lots to say).
When you complete the survey you will be entered twice into a drawing for a $10 gift card. Your chances of winning a gift card is approximately 1 in 10. If you exit the survey before it is complete you will be entered into the drawing once. Be sure to provide your email address or phone number below to be able to claim your prize should you win a drawing.
On the next page you will find more information about the research study and be asked to provide your voluntary consent to participate in the research.
