Thank you for using the newly designed referral template.

This template was developed to make it easy to provide important information in the referral process. The referral letters can now auto-populate fields using information available in your EMR.

This referral form was adopted from the template collaboratively designed by the College of Family Physicians of Canada and Royal College of Physicians and Surgeons. It has been endorsed for use in Saskatchewan by the Council of the College of Physicians and Surgeons of Saskatchewan, the provincial Senior Medical Officer Committee and the Saskatchewan Medical Association.
