This form is for those that are requesting transportation services. PLEASE SUBMIT ONE FORM PER STUDENT. PLEASE NOTE: Students with Special Needs will have their transportation services set up through their assigned schools or departments.

To be eligible to ride a school bus, students must live at least one and one-half mile from the school which they attend unless the situation is deemed hazardous by the director of transportation.
