CPV Pre-Training Survey

General Information

This page gathers your personal information so that we are able to track those who will use the College Positive Volunteerism (CPV) curriculum. This survey is designed for those who are planning to attend a training to be certified as a CPV. All of the information that you provide will remain anonymous. The data is used to improve the program. Thank you so much for taking the time to fill this out!
