You are being asked to complete this survey because you attends a school where the REACH Beyond Domestic Violence agency implemented a dating violence prevention activities this year. This survey will take you about 10-15 minutes to complete. We know how busy you are: please know that by filling out this survey you are doing us a tremendous favor. Please also note: you may skip questions that you do not want to answer.
REACH has been asked to evaluate its success. One way to evaluate success is to ask students to complete a survey at the beginning of the year, and then again later in the year, in order to find out if there are differences. Therefore, you are being asked to complete this survey to help REACH understand whether its programming has an impact.
This survey is anonymous and confidential. Please answer as truthfully as possible. Answers will be aggregated--no individual's responses will be analyzed. You will NOT be judged on how you answer! Please feel free to be completely honest.
This is an evaluation survey, not a formal research project. Therefore, this survey has not been reviewed by a Human Subjects Committee and there is no consent form. The information gathered will be used to determine if REACH services are working and/or how they can be improved.
