You are invited to complete this survey because of your connection with Fourth Presbyterian Church.  This survey is being conducted by the Racial Equity Council (REC) of Fourth Church.  The purpose of the REC is to help the Session of Fourth Church lead the church to become a fully multicultural, antiracist organization. The aggregate responses from the survey will help the REC develop recommendations to the Session.

Systemic racism is defined as race prejudice combined with the misuse of power by systems and institutions.  Antiracism work at Fourth Church includes removing barriers and obstacles that prevent full participation of people of color, and changing practices and policies that benefit only white people and/or harm people of color.

This survey will take about 10 minutes to complete.  We value your time and honest opinion. No attempt will be made to connect your answers with you as an individual, unless you choose to share your name. We invite you to answer several demographic questions so we can better understand if perceptions differ for people in different demographic groups. This information is confidential.
