Learning Labs

The Center for Quality Improvement & Innovation (CQII) has launched a new virtual QI training program, called QI Learning Lab. It is comprised of multiple independent Labs, targeting different quality improvement (QI) capacities of training participants. Each Lab lasts consists of an orientation/pre-work session, followed by six 90-minute virtual sessions every two weeks over the course of three months. All participants, regardless of the Lab, register here using this application portal. We request a variety of information from you, including demographic and contact information, your individual commitment and level of agency leadership support to complete all Lab assignments, current and past QI projects, personal goals for completing the Lab, and an assessment of your individual QI proficiency. Advanced Lab applicants are asked to submit additional supporting documents (e.g., recent QI storyboards) to further evaluate their past experiences.

Take the time to appropriately respond to each question. Please note that not having previous quality improvement training or experience will not prevent your or your organization from joining the QI Learning Labs. Rather, the information you provide is used to ensure you are placed in the most appropriate Learning Lab.

If multiple staff members of the same agency jointly apply to participate as one QI team in the same Lab, please ask each QI team member to independently complete this survey.
