Home First Carers Survey |
This survey is designed to gather feedback from people who are caring for someone who has recently been discharged from a hospital across Bristol, North Somerset and South Gloucestershire. You may be a formal carer or a family member, friend or neighbour who is supporting someone in the short-term after leaving hospital.
The survey is being managed by the Bristol, North Somerset and South Gloucestershire Integrated Care Board (ICB), in partnership with other members of the Integrated Care System.
The responses to this survey will be used to improve the service and information we provide in the future to people who are being discharged from hospital, and their carers. The answers you give to this survey will be anonymous and confidential. The Bristol, North Somerset and South Gloucestershire ICB privacy policy will apply to the answers you give and your answers will also be shared with Sirona care & health.
